I think I've made so many mistakes that I can't count them. I do try and not make the same mistake twice. But new ones keep popping up!

Normally, a bad decision involves a mistake or is a mistake.
As an example, recently when I broke my arm,
it occurred because of a mistake AND a bad decision.
My mistake was a mis-step backwards that resulted in a fall.
My bad decision was to concentrate on something in front of me,
and neglect to pay closer attention to my own movements.
