I think keeping the handle of the hammer parallel with the forearm is how most people do it. Of course you could try various other grips to challenge your hand/wrist more.

I keep the hammer parallel with my forearm. I just have the end of the handle resting against my elbow so my wrist is not supporting the "levering force" of the hammer.

That style would probably allow you to put more stress on the bicep and less on the forearm, if that's what you want. I don't actually do flip lever very much since it's time consuming and I think other movements offer more bang for my buck.
